Sample Forms
In the following pages you will find samples of the forms and documents you will be using in your work as a mortgage broker. Take a few moments to review and become familiar with the language and requirements of each form. You will not have to generate any of these forms, as they are part of the mortgage broker software packages.
This form is completed by the borrower and details his employment history, income, debts, assets, and liabilities.
The mortgage broker provides the borrower with an estimate of the charges he will likely incur at the loan settlement.
This form, with applicant information and property description, is submitted to the underwriter of the lending institution.
Signed by the borrower, this form gives the mortgage broker authorization to verify the information the borrower submitted in support of his loan application and also authorizes a consumer credit report.
Signed by the borrower certifying that the information provided in support of the loan application is true and complete, and authorizes release of information.
Form submitted to the Internal Revenue Service to request tax return information.
A pre-application form that is full disclosure of the application process and fees.
