Business Continuity Planning Assessment Questionnaire

Use the following survey to assess your organization’s current level of business continuity preparedness. For each statement, circle the score from 1 to 5 that you believe best reflects what is in place today, 1 being the lowest score, 5 being the highest score (1 = nothing currently in place; 5 = fully implemented). Note: If you do not know whether something exists, for purposes of this assessment, it does not.

There is a great deal of work to be done. Get started now. Develop a comprehensive project plan; involve people from all areas of the organization. Find an executive sponsor for the project.
