THE DISCIPLINARY ACTION FORM

At certain times, you may find it preferable to use an action form, rather than a letter, to document the disciplinary-process step you are taking. I personally prefer it because it adds structure, carries a formal title, and acts as a memory jogger for the various steps of the discipline process.
