Incident Description and Supporting Details: Include the following information: Time, Place, Date of Occurrence, and Persons Present as well as Organizational Impact.
Ingrid,
On October 10, you reported that you lost your company hard hart while visiting a customer’s premises. This document serves to notify you that you are being issued a written warning for violation of the Company Issued Tools & Equipment policy, which follows:
Employees are required to use company tools, equipment, property, and resources safely and responsibly and to use good judgment in maintaining the security of company-issued equipment. Employees in positions requiring the use of company tools and/or equipment are responsible for their care and safekeeping. Loss or damage to company property many result in corrective action, up to and including termination of employment.
You reported the misplacement/loss of your hard hat to your supervisor in a timely manner and stated that you were working on an electrical outage issue with another technician. You confirmed that the hard hat was not necessary as personal protective equipment (PPE) for the repair you were conducting and that you needed to remove it to fit into the space. However, you also stated that at the time, you did not realize you left your hard hat in the rear of the vehicle. Further, as you proceeded to drive away, you came to a stop sign and realized your hard hat wasn’t near the docking station where you normally keep it. You searched your company vehicle but could not find the missing hard hat—you also called the other tech to help search the location where you had been working, but you were unable to find it.
As a result of not following the Company Issued Tools & Equipment policy, you are being issued a written warning, which affords you the opportunity to correct this behavior.
1. Measurable/Tangible Improvement Goals: Ingrid, the proper care for your company-issued tools and PPE is an area that you must monitor on a regular and ongoing basis and is an essential function of your job as a field services technician. Be aware of your surroundings and never leave any company property unattended, out of sight, or where damage or theft may occur.
Remember to properly store/secure your equipment, carry it with you as you go from site to site, and account for it at the completion of every assignment. Likewise, conduct a quick tool/equipment check before and after completing each job and at the beginning and end of each workday, to ensure you have all of your tools and equipment accounted for.
2. Training or Special Direction Provided: To emphasize the importance in handling company tools and/or equipment, please refamiliarize yourself with our company’s “Issued Tools & Equipment” policy. Further, please review our policy on Personal Protective Equipment immediately. You are required to review all company policies and make yourself familiar with them on a regular, ongoing basis. Please ask your supervisor, manager, or HR support person any questions if you are in need of any clarification on this or any other policy.
3. Interim Performance Evaluation Necessary? No
4. Our Employee Assistance Program (EAP) provider, Prime Behavioral Health Group, can be confidentially reached to assist you at (800) 555-5555. This is strictly voluntary. A booklet regarding the EAP’s services is available from Human Resources.
5. In addition, I recognize that you may have certain ideas to improve your performance. Therefore, I encourage you to provide your own Personal Improvement Plan Input and Suggestions:
(Attach additional sheets if needed.)
Positive: I will remain available to help you and discuss areas where you require additional support. If you meet your performance goals, no further disciplinary action will be taken regarding this issue.
Negative: You must clearly understand that failure to demonstrate immediate and sustained improvement may result in further disciplinary action up to and including dismissal. A copy of this document will be placed in your personnel file.
